How support begins

Getting support should feel clear from the first conversation.

When a person or family reaches out, our first goal is to understand the need, explain available Support Solutions services, and begin a path toward care that fits the person and their community.

We learn about the person

Tell us about the person's needs, goals, daily routines, health and safety concerns, and the support they are looking for.

We explain service options

Our team helps determine which Support Solutions services may fit, what information is needed, and what availability looks like in the person's area.

We begin the next step together

If Support Solutions is a fit, we work with the person, family, and care team to move toward intake, planning, and support.
