How support begins
When a person or family reaches out, our first goal is to understand the need, explain available Support Solutions services, and begin a path toward care that fits the person and their community.
Tell us about the person's needs, goals, daily routines, health and safety concerns, and the support they are looking for.
Our team helps determine which Support Solutions services may fit, what information is needed, and what availability looks like in the person's area.
If Support Solutions is a fit, we work with the person, family, and care team to move toward intake, planning, and support.
A different standard of care
Excellent support is not only visible in a treatment plan or an accreditation badge. It shows up when someone chooses dinner, learns a bus route, gets ready for work, calls a friend, settles into a safe home, or feels respected during a difficult day.
We design support around the person. Services flex around goals, preferences, strengths, health needs, and communication style.
We make independence practical. Support is focused on daily skills, routines, employment, relationships, and community access.
We stay steady through complexity. Families and referral partners need a team that does not disappear when support gets hard.
